Our carers work alongside district nurses, GPs, hospice teams, and other healthcare professionals to ensure that every aspect of care is coordinated, compassionate, and centred entirely on the person's comfort and wishes.

Macmillan Nurses — respecting and reinforcing the specialist support they provide
St Luke's Hospice at Home team — coordinating visits to complement hospice support
GP and specialist consultants — acting on updated care plans promptly
NHS Continuing Healthcare teams — supporting reassessments and transitions
